✈️ Flying Sober Shouldn’t Mean Going Broke: The Financial Crisis Facing Pilots in HIMS

When you step onto a plane, you trust that the pilot is smart, trained, and medically clear. What you probably don’t see is the enormous financial stress many pilots face while navigating the FAA’s HIMS program. This system is designed to help—but right now, it’s often hurting.

💸 The Cost of Staying in the Sky

Pilots entering HIMS don’t just go through treatment and monitoring—they face massive out-of-pocket expenses:

  • Treatment & Evaluations: Inpatient or outpatient rehab costs tens of thousands. Add neuropsych testing and FAA medical exams, and bills can easily climb above $20,000.
  • Ongoing Testing: Monthly drug/alcohol screens, AME visits, sponsor sessions—hundreds per month.
  • No Pay While Grounded: Pilots are often removed from flight status for 6–12 months—or longer—with little to no income.

For a six-figure pilot, total costs—including lost salary—can hit $100,000–150,000 within a year.

🚨 Why So High?

  • No Insurance Coverage for FAA Requirements: Even with solid insurance, many HIMS-mandated neuropsych tests and monitoring aren’t covered.
  • Lack of Employer Support: Few airlines or unions step in to cover these costs, leaving pilots to foot the bill.
  • Culture of Silence: The stigma—and high financial barrier—discourages early self-reporting, leading to worse outcomes and higher costs.
