Yesterday, we published a report estimating 5,000 to 6,000 open cases within the FAA’s Aerospace Medical Certification Division (AAM300). That estimate was based on internal records and prior disclosures.
However, new information now confirms that the real number of backlogged cases is over 55,000—including medical certificate reviews, SSRI cases, HIMS-related evaluations, and other certification queues across all classes of airmen.
This staggering backlog reportedly surged after a wave of staff reductions that gutted the FAA’s processing capacity. The result: thousands of pilots and air traffic controllers remain in indefinite limbo—not because of safety concerns, but because there is no one available to review their files.
The Human Cost of Bureaucratic Collapse
- Pilots with clean medical histories are grounded for months or years.
- Applicants face out-of-pocket costs of $10,000–$20,000+ for FAA-required testing and evaluations.
- Flight currency is lost due to months-long deferrals with no updates.
- Some professionals are unable to return to work, despite being fully qualified.
Many of these individuals have no disqualifying condition. Their paperwork is simply waiting—often unread—because the FAA lacks the capacity to process it in a timely manner.
System Failure Is Not Acceptable
Medical certification should be based on facts, timelines, and fairness—not silence, opacity, and delay. The current backlog is a threat not only to individual careers, but to the aviation system itself. What’s needed now is urgent reform, accountability, and transparency from the FAA. We must end the practice of indefinite holds, create a tracking system for applicants, and restore trust in the medical certification process.
