Why HIMS Testing Should Include Thebaine
The FAA’s own toxicology research has already proven that omitting Thebaine from opiate panels risks false positives. In a 2005 study conducted by the FAA’s Civil Aerospace Medical Institute (CAMI), scientists identified Thebaine as a reliable marker of poppy-seed ingestion—not drug abuse. The report, Poppy Seed Consumption or Opiate Use: The Determination of Thebaine and Opiates of Abuse in Postmortem Fluids and Tissues (DOT/FAA/AM-05/11), established a method to simultaneously test for eight compounds, including morphine, codeine, and Thebaine.
Thebaine is a natural opiate alkaloid found in poppy seeds. When someone eats a poppy-seed bagel or muffin, trace amounts of morphine and codeine can appear in urine or blood tests. However, Thebaine appears only through poppy-seed consumption and never through prescribed or illicit opiate use. Detecting Thebaine therefore allows laboratories to distinguish dietary exposure from actual drug abuse.
In the FAA’s own published findings, investigators analyzed eight aviation fatalities and confirmed that the presence of Thebaine—alongside low levels of morphine and codeine—was consistent with poppy-seed ingestion, not narcotic use. The study explicitly concluded that “a subsequent Thebaine positive would indicate poppy seed consumption and could possibly rule out opiate use.”
Despite this, current HIMS testing panels do not include Thebaine. This omission means pilots and controllers can test “positive” for morphine or codeine after eating perfectly legal foods, with no way to prove dietary origin. Such results can trigger unnecessary investigations, treatment mandates, and professional harm. The FAA’s own forensic guidance recognizes this flaw, yet the operational HIMS program continues to rely on incomplete testing standards.
It’s time for consistency between science and policy. If the FAA’s toxicologists already validated Thebaine as the key differentiator, HIMS laboratories should be required to include it in their confirmation panels. Doing so would protect innocent airmen from career-threatening false positives, uphold scientific integrity, and restore trust in the medical certification process.
The evidence is clear: when Thebaine is ignored, truth is lost. When it’s included, fairness is restored.
