Efforts Toward Reform and Transparency

Note: Reform efforts now reflect the expanded reach of the FAA HIMS Program—affecting not only pilots with substance use histories, but also those with SSRI prescriptions, trauma, anxiety, therapy records, or no diagnosis at all. Advocacy includes all such pilots under psychiatric oversight.

Reform and Advocacy in the HIMS Program

The original intent of the FAA HIMS Program was to support pilots in recovery, not to create indefinite barriers to reinstatement. In recent years, however, pilots, physicians, legal experts, and aviation unions have voiced growing concern about systemic problems within the program.

Multiple stakeholder groups are advocating for reform in areas such as:

  • Transparency: Pilots often receive decisions with no stated rationale. Advocates want clearer guidance on what is required to exit HIMS or obtain unrestricted certification.
  • Individualization: A one-size-fits-all approach to recovery fails to account for pilots’ diverse medical and life circumstances.
  • Oversight and Accountability: FAA medical consultants operate behind closed doors. Reform efforts seek independent review panels and appeal rights for medical decisions.
  • Science-Based Standards: Advocates push for updated policies that reflect modern addiction science, relapse risk assessments, and peer-reviewed data.

These efforts are not anti-safety — they are pro-accountability. Pilots are calling for a balanced system that ensures aviation safety while respecting medical privacy, due process, and the human dignity of those in recovery.
